Registration:

Please download the registration forms for your child(ren) here. Completed registration forms are required for all children not currently enrolled in any Clifton FUMC Preschool program. These forms are required for admission into our program and must be updated on a yearly basis in order to comply with licensing standards. Please note that all forms of the registration packet linked above must be filled out completely and signed and know that several of the pages will require the signature of your child’s physician. You may attach a copy of your child’s current vaccination records to the Admission Form in the packet.

Registration fees are to be paid at the time of initial registration and again during annual re-registration, even if registration is in the middle of the year. The registration fee is $80.00 annually.

• Payment of a Registration Fee does not guarantee your child a place in our Preschool Program.

• Fee payment must accompany your child’s completed registration forms and be given to the Preschool Director in person.

• Registration fees are to be paid once a year at the time of initial registration and again during annual re-registration, even if registration is in the middle of the year.

• All registration and supply fees are non-refundable.
